Application Information
To begin the application process you will need to sign and return the following items:
- Student Application Form, signed in two places
- Application/Enrollment fee
- Copy of Birth Certificate
- Current Florida Immunization Record (must be the original blue 680 Form)
- Proof of current health exam (within one year)
- Most recent report card
- Transcript for grades 9-12, test (e.g., SAT), address of transferring school
- Discipline record from previous school
- Pastor's Recommendation Form
- Current Individual Education Plan, if applicable
- Psychological evaluation, if applicable
- $25.00 entrance placement/testing fee (all new students)
- Emergency Treatment Form
- Permission to Administer Medicine, if applicable
- Computer/Internet User Agreement and Permission Form
- FACTS Form
- Driving Permission Form, if applicable
- Parental/Guardian Statement of Cooperation and Obligation Form (to be submitted after reading the Handbook)
All applicants in grades K5-12 must schedule an interview with the appropriate school administration. If placement testing is necessary, an appointment will be scheduled.
Note: When you return your Student Application with the above items, you will be given a Student Family Handbook, which begins with an introductory letter and the Parental/Guardian Statement of Cooperation and Obligation Form. This form must be signed and returned before your child's application can proceed.
